Architectural Modification Request Form (AMF)

Required: You must provide any of the supporting documentation listed below that applies to your request with this application. If you submit an incomplete application, the AMFC will not review your proposal or make a recommendation to the Board until it is complete.

Paint/stain colors: A sample, brand and model number of the color.
Finish materials: Description and/or sample of materials for the exterior.
Photographs: Exterior lighting fixtures, decorative objects, and any other cosmetic additions.

Notes of Agreement:
A. When required, owner must obtain building permit for City of Sioux Falls.
B. Owner understands that they cannot start any work until they receive written approval from the Board.
C. Owner agrees that if any exterior alterations are started before receiving approval, they may be required to restore their property to its former condition at their own expense and pay all associated legal expense.
D. Owner agrees to allow AMF committee or Board member permission to inspect modification of property.
E. Owner agrees to complete improvements in a workmanlike manner and in accordance with the approved specifications.
F. Owner understands that the approval for this improvement will expire if not started within 180 days following approval.
G. Owner understands that they must complete this improvement within 60 days of starting the work, unless the Board authorizes another time frame in writing.
H. Owner is responsible for repairing any damage to the building exterior, roofs or common areas resulting from work performed to the satisfaction of the Association.

Please include the type, color, or material planned; who will be doing the work; and other pertinent information. The Architectural Modification Form Committee (AMFC) and the Board will need to evaluate your request.
